PPO plans give you the freedom to see any doctor — in Florida or anywhere in the country — without a referral. Florida Blue BlueOptions is the only statewide PPO in Florida's individual market. Philip Smith compares PPO vs. HMO vs. EPO based on your doctors, budget, and how you use healthcare.

Florida Blue's BlueOptions plan is the only PPO available statewide in Florida's individual and family health insurance market. Other carriers — UnitedHealthcare, Cigna, Aetna, Oscar, Ambetter — offer HMO and EPO plans in Florida, but none offer a true PPO with out-of-network coverage.
BlueOptions includes the BlueCard program — a nationwide reciprocity network shared by every Blue Cross Blue Shield affiliate in all 50 states. When you see a doctor in another state on BlueOptions, you pay in-network rates, not out-of-network rates. For Floridians who travel, work remotely, or spend part of the year in another state, this is a critical distinction.
BlueOptions is also the most common PPO choice for self-employed Floridians earning above the ACA subsidy threshold — because they pay full premium and the flexibility of a nationwide network is often worth the premium difference over an HMO.
2026 Florida PPO Premium Increase
Florida PPO premiums increased an average of 31.5% for 2026 after the enhanced ACA premium tax credits expired in December 2025. If your current PPO premium increased significantly, Philip can compare whether a private medically-underwritten plan (available year-round, not ACA-regulated) would cost less for your age and health history. Many healthy Floridians save $200–$400/month by switching from an ACA PPO to a private plan.
See 2026 Florida rate guideFlorida Blue BlueOptions is the only carrier offering a true PPO plan statewide in Florida's individual and family market. BlueOptions gives you in-network access to Florida Blue's statewide network plus out-of-network coverage nationwide through the BlueCard program — which means you can see any BCBS-participating doctor in all 50 states. UnitedHealthcare and Cigna offer EPO plans in Florida (no out-of-network coverage) that are sometimes marketed as PPO-style. A licensed agent will explain the exact difference based on your zip code and doctors.
Florida PPO premiums increased an average of 31.5% in 2026 after the enhanced ACA subsidies expired in December 2025. For a 40-year-old non-smoker, a Florida Blue BlueOptions PPO runs approximately $550–$750/month before any tax credit. If your income is below $62,600 (single) or $127,000 (family of four), you may qualify for a subsidy that significantly reduces this. For healthy adults above the subsidy threshold, medically-underwritten private plans can cost $266–$400/month — Philip will compare both options at no cost.
A PPO is right for you if you have established specialists you want to keep, travel or spend time in another state, are self-employed with the flexibility to see any doctor, or want nationwide coverage without referrals. An HMO is right if you want the lowest possible monthly premium and are comfortable staying in-network with a primary care physician gatekeeper. Florida Blue's HMO plans cost significantly less than BlueOptions PPO — for many Floridians who rarely leave the state and don't see specialists frequently, the HMO math wins. Philip will run both comparisons for your exact situation.
Yes — Florida Blue BlueOptions PPO includes BlueCard nationwide coverage. BlueCard is a reciprocity network shared by all Blue Cross Blue Shield carriers across every state. When you use healthcare out of state on a BlueOptions plan, the local BCBS carrier processes your claim as in-network, so you pay in-network rates. This is the primary reason snowbirds, frequent travelers, and people with specialists in other states choose Florida Blue BlueOptions over other plan types.
It depends on how you use healthcare. If you have a chronic condition requiring specialist care, see doctors in multiple states, or have family members with established provider relationships, the extra cost of a PPO is typically worth it. If you're healthy, rarely see doctors, and stay in Florida, an HMO or EPO plan saves you $100–$300/month for the same basic protection. Philip will calculate the actual total cost (premium + expected out-of-pocket) for both options based on your health history and typical usage.
